Immunization Requirements- Incoming 7th Graders

In an effort to provide the best quality service to families new to our area, we ask that you please have your child’s registration packets completed prior to your appointment.

Please remember that Florida State Law requires that your child to receive a TDAP immunization prior to entering 7^th grade.  Please bring in their immunization record so your child is able to receive their class schedule.  Listed below is our local health department information or see your child’s pediatrician before the start of the school year.
